I cannot remember where or how I found the meditation, so I can't credit anyone on this. All I know is that this meditation is extremely old, has great benefits, and serious risks.
DO NOT ATTEMPT IF: you suffer from any sickness or disease that affects your breathing capabilities, if you suffer from asthma, or have suffered a heart attack recently.

Rules: You must perform this meditation when you are alone. There MUST not be anything that could make you lose focus for even a second. Absolute concentration is required - no distraction inside & outside of your mind.
You must lay flat on your back as comfortable as possible - even the slightest irritation or pain could destroy your focus. And above all else, you MUST have self control - do not freak out if you mess up.
Optional - Perform between 2am & 4am when all energies, be it positive or negative are at their weakest.

Lay on your bed or couch, arms an legs sspread out, your body must remain still. Clear your mind of absolutely everything before proceeding.
Do not listen to music of any kind, even if its just plain sounds, if you cannot meditate without them, then you will never be able to complete the meditation.

After clearing your mind, shorten your breathing dramatically - imagine your regular breathing in terms of size, your regular breaths are the size of a watermelon, you have shrink it to the size of a golf ball. Once you have managed to keep at this breathing level, wait for a while for your body to show you its working.
Your body will begin to numb - DO NOT LOSE FOCUS! This is normal, your body isn't getting enough an is attempting to adjust, continue the breathing an let the numbness continue. Soon your entire body will become partially paralyzed an soon the numbness will reach to the back of your skull.
Remember, you must NOT lose focus.

If you do however lose focus, you must bring your breathing level slowly back up! If you suddenly start breathing too rapidy, your body WILL freak out an it will NOT be pleasant. The least of your worries is that you'll begin to hyper ventilate, vomit, or simply faint. The worst that can happen is that you'll either experience a heart attack, you'll fall into a coma, or you'll experience a violent seizure - Hence why you MUST NOT LOSE FOCUS!

Continuing on, once the numbness reaches the back of your skull, close your eyes but DO NOT FALL ASLEEP! During this point, you must keep up the breathing & focus the entire time.
Now this will sound a bit weird so bare with me. Imagine that you are standing in complete darkness, look down an you'll see a flight of stairs going downward, go down the stairs. At the the bottom of the stairs will be two large doors (wooden or metal, modern or medieval), and they are locked.
Open your hand, you have the key, unlock the doors, open them an step through them. Inside you will find a large room that looks like a library, the rest is up to you.

I cannot tell you what it will look like, how many books there, or how many, if any, floors there are. You have stepped into your own essence, a place only you can access. In this library, you will find ALL of your memories, your past experiences, some memories will not be apart of your current life time, but there are books that will unlock what your essence already knows but what hasn't been known to you in your physical.
Think of these books as pictures an you have Amnesia, pictures help you remember who you are. Use this time WISELY as you cannot keep up this meditation forever an it will require multiple meditations to remember everything. Also note that time moves different while you are in your essence, whereas you know its been at least an hour inside your essence, only a few minutes have past according to your body.

Another quick note, if you see anything that resembles a robed figure, then it is time to exit as soon as possible (your body will be slowly dying from the lack of oxygen it requires to remain alive, an that figure means that if you do not end the meditation soon, it will take you an you'll die in the process).
Once you leave the library, close the doors, they'll relock on their own, and now open your eyes. Bring your breathing level slowly back up to normal an then let your focus diminish, your body will begin lose the numbness, and after that, well you've managed to perform the Necromancer's Meditation.

About memories.

According to psychology, memories are very fallible, some can be based on events that never have happened (known as false memories).

So how does one know what one sees in this library be actually accurate or is there a way around this flaw of the human mind through magical measures?

When you dive into your own mind, more specifically your true soul/self, everything in the library is based on YOU an you alone.
The flaws bit I cannot answer because the library is built an made up of how you see the world around you. Do you love challenges? The library will present them. Do you enjoy art? Art in all forms will be littered all across the library as either a mess or in a categorization. Are you true to yourself? The library will shape it by how much truth you have an how much lies you have.

It can have any number of floors, it can be filled with your imagination can achieve, an if you don't have much control over yourself, there might even be barriers in the library, puzzles, things that'll delibrately break your concentration, the library is essentially a part of you that was never designed for self immersion. It contains all the knowledge that your true self knows! And yes, sometimes you will given bits that are either half truths or nothing at all, you just have to know that it is you, the library is you, if you don't believe something is there then it won't be there.

Think of it as if you are in a holodeck (star trek), you are shaping everything to your heart's desire, but in this case without a console or even saying anything to some computer an there are no rules inside yourself unless you personally put them there yourself.

The best way in my opinion to go about it is simply think of every single book as a picture an you have amnesia and you are trying to remember who you are. Do that an your visit to your own library will be that much easier.
Ignore reality, Ignore what you think is plausible an what is not, Ignore what your shell has learned & remember what your true self knows!

Are there limits to what you can do? Your physical self may think "Yes" but your true self will say "There are no limits, limits is an illusion".
And I guess there is another bit I think I should have noted, you have to want to do this, you have to want to know the truth, cause if you don't, then you will never know it until you have the need or desire for the truth.
